CMS 1500, lines 19 and 27

In CMS 1500 for lien-based, personal injury cases, I have heard 2 different versions about lines 19 and 27.
1. Leave line 19 empty and check "yes" for line 27.
2. Check 'No" in line 27 and put something like this in line 19: FOR DOCUMENTATION ONLY, NOT SUBMITTED TO HEALTH INSURANCE. TREATMENT PROVIDED PURSUANT TO MEDICAL LIEN/LETTER OF PROTECTION. PATIENT REMAINS RESPONSIBLE UNDER THE EXECUTED AGREEMENT."

Which one is better?
 
That's a real fork to be stuck on since those two versions basically send opposite signals to whoever processes the claim. Lien cases seem like they'd need extra care to keep the paper trail straight. Have you run into this with a specific payer or is it more of a general office policy question?
