Questions on 77305 and what counts toward complex

Trying to make sense of 77305 and when it's considered complex. If the patient's file includes things like hyperthermia probe verification or contrast use, does that automatically qualify or does it come down to whether it was performed during the session? Also where can I find the exact documentation specs for those items?

Thanks for taking the time to respond. The CPT manual is where I started but it leaves a lot of room for interpretation on what pushes a plan from basic to complex. I'm really just trying to dot all my i's and cross my t's on this one by seeing how others handle it.
