Medicare coverage for Tier 2 lab 81403?

I have a quick question. if a lab order shows up as a misc test, and it's tier 2 test, will this be covered by Medicare? For reference, it's for lab 81403. Can anyone confirm this or share any codes that worked for you that might help get it paid? Thanks so much!

81403 is on the Tier 2 molecular pathology list, so Medicare usually covers it if the diagnosis code backs it up.
